Erie Community College Sports Medicine
The Sports Medicine staff at SUNY Erie Community College is comprised of two Certified Athletic Trainers (ATs) who work in conjunction with our Team Physicians of UBMD Orthopaedics and Sports Medicine.
ATs are highly qualified, multi-skilled health care professionals who render service or treatment, under the direction of or in collaboration with a physician, in accordance with their education, training and the state's statutes, rules and regulations. As a part of the health care team, services provided by ATs include primary care, injury and illness prevention, wellness promotion and education, emergent care, examination and clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions. Athletic training services are available to all current intercollegiate student-athletes at SUNY Erie Community College.
Pre-Participation Paperwork:
On behalf of the Erie Community College Sports Medicine Staff, we would like to welcome you to campus as a prospective member of our athletics program. In order to comply with NJCAA health and safety regulations, there are multiple forms all incoming and returning student athletes must complete prior to participation in any athletic-related activity at SUNY Erie Community College. All pre-participation forms on the ATS Athlete Portal, Physical, and Liability Waiver need to be reviewed before one's sport participation.
Requirements for Athletic Participation:

Instructions for completing the athlete profile in ATS.
• Complete all “required” fields and forms.
• Must be completed prior to participation by all student-athletes.
2. Physical exam completed on the SUNY Erie Community College Physical Form (PDF) and additional forms as needed
• ***Physicals completed on other forms will not be accepted.
• Previously accepted physicals will expire 13 months from the date of the exam.
• Completed physical forms must be turned into the Sports Medicine staff either in person or by fax at (716) 851-1287.

4. Proof of Insurance Uploaded to your ATS Athlete Portal
• Upload photos of the front and back of your insurance card
5. Attendance of a Pre-Participation meeting with the Sports Medicine Staff

6. Please bring an extra dose of any emergency medications (Rescue Inhaler, EpiPen, etc.) that you are prescribed and documentation of your sickle cell status (if you have any).
**Failure to comply with all the above stated Sports Medicine requirements will result in a delay of your medical clearance to participate in athletics.
